Medication at School      

Medications should be clearly labeled with the following information:

• Childs Name 
• DOB
• Medication Name
• Dosage and how to take

We also require an Administration of Medication Form to be completed and signed by the parent/carer. Please ensure the medication is delivered to the school and given directly into the care of administration
